Nicotine pouches, such as Zyn, On!, and Velo, have rapidly gained popularity among U.S. adolescents. These products are small, discreet, and often mistaken for candy due to their inconspicuous, brightly colored packaging. Aggressively marketed on social media with youth-targeted imagery and memes, nicotine pouches are perceived as trendy and low-risk despite delivering highly addictive nicotine. National surveys reveal a sharp increase in youth use: the 2024 National Youth Tobacco Survey found 480,000 current adolescent users, doubling since 2021, with over 30% of middle and high school students aware of these products. Use is most common among male, White, and rural youth and frequently occurs alongside vaping or traditional tobacco use.<br /><br />The literature review highlights the role of appealing flavors, "tobacco-free" labeling, and discreet design in underestimating harm among youth. Nicotine pouches are placed between the lip and gum, providing oral nicotine absorption without tobacco. This new trend raises significant public health concerns due to the addictive nature of nicotine and the normalization of use through social media promotion.<br /><br />Clinicians often do not distinguish nicotine pouches from other nicotine sources, leading to missed opportunities for screening and intervention. Importantly, there are currently no evidence-based cessation treatments tailored for adolescent nicotine pouch users. Although therapies like Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and Nicotine Replacement Therapy (NRT) are used for nicotine dependence, their effectiveness specifically for pouch cessation remains unstudied.<br /><br />This gap underscores the urgent need for research to develop targeted interventions and public health strategies addressing adolescent nicotine pouch use. Screening, counseling, and education must incorporate recognition of these products to combat their rising popularity and potential addiction risks among youth.
