Trends of prescription stimulant and cannabis use in patients hospitalized for psychosis and mania

This study examines trends in prescription stimulant and cannabis use among patients aged 18-35 hospitalized for psychosis or mania at Mount Sinai hospitals between July 2020 and September 2023. Background data show a 58% rise in prescription stimulant use nationwide from 2012-2022, associated with a threefold higher risk of psychosis or mania, and a significant increase in daily cannabis use, which has surpassed alcohol use since 2022 in the US. Cannabis is linked to increased psychiatric hospitalizations, mania risk, and possibly 8-13% of schizophrenia cases.<br /><br />In a retrospective review of 264 inpatients, the sample was 59% male, average age 26, primarily diagnosed with psychotic spectrum illnesses, with an average 12-day hospital stay. Cannabis use was reported in 61% of patients—three times higher than the US average of 20%—and prescription stimulant use in 11%. Cannabis users were younger by 1.5 years on average and nearly twice as likely to be male. Prescription stimulant users were more associated with bipolar or substance-induced disorders, though differences by diagnosis and sex were less significant.<br /><br />No statistically significant increase in cannabis or prescription stimulant use was observed across the studied years, though there was a marginally significant rise in cannabis use between 2020 and 2021. The proportion of cannabis use in this inpatient population remains noticeably elevated compared to national averages, but this study did not find a corresponding upward trend over time mirroring national increases. Possible reasons include higher-functioning stimulant users not requiring hospitalization and limitations such as single-site data, small stimulant user sample size, charting inaccuracies, and confounding polysubstance use.<br /><br />The findings highlight the high prevalence of cannabis use in psychiatric inpatients with psychosis or mania but suggest trends in stimulant and cannabis use among these patients differ from broader population trends. Future research could explore differences between emergency and inpatient populations and improve substance documentation methods.
