Rising Burden of Substance Use Disorders in High-Income North America

This study investigates the rising burden of Substance Use Disorders (SUD) in High-Income North America (HINA), encompassing the U.S., Canada, and Greenland, using Global Burden of Disease (GBD) 2021 data from 2010 to 2021. Despite stable incidence rates (Estimated Annual Percentage Change [EAPC] 0.50), both prevalence and disability-adjusted life years (DALYs) due to SUD have significantly increased, especially in the U.S., where prevalence rose from approximately 4,597 to 5,675 per 100,000 (EAPC 2.12) and DALYs nearly doubled from about 1,264 to 2,244 per 100,000 (EAPC 5.51). Canada showed a moderate DALY increase (EAPC 3.23), while Greenland experienced a decline (EAPC -1.20).<br /><br />Age- and sex-specific analyses reveal that males, particularly young adults aged 15-29, bear the greatest burden. Compared to global and other high-income regions, HINA reports the highest SUD prevalence and DALYs. Within the U.S., states in the western and southwestern regions, such as Arizona, New Mexico, Alaska, and Nevada, exhibit the highest SUD rates, contrasting with lower rates in southeastern and midwestern states like Mississippi and Alabama.<br /><br />Global comparisons confirm that high-income regions have among the highest SUD-related burdens, with HINA showing the steepest rises since 1990. This pattern highlights critical demographic and geographic disparities essential for guiding tailored prevention efforts, clinical interventions, and health policies.<br /><br />The study calls for urgently enhanced, age- and sex-specific strategies, including integrated addiction and mental health services, and policies addressing social determinants of substance use. Without targeted interventions, the rising SUD burden—particularly in young to middle-aged males and high-burden states—will continue to strain public health systems in HINA.
