Physician Health Programs A Brief Review of Successes and Opportunities

This narrative review examines Physician Health Programs (PHPs) as effective interventions for physicians with substance use disorders (SUDs) or other impairing conditions, based on studies and surveys from 2008 to 2023. With rising national prevalence of SUDs, addressing their impact on physicians is crucial. PHPs provide an evidence-based recovery approach, with long-term abstinence rates surpassing those of the general population. A retrospective cohort study involving 904 physicians across 16 PHPs showed that nearly 79% remained licensed and practicing five years post-enrollment, and 81% completed the program without relapse. Additionally, satisfaction surveys indicated high approval, with 92.5% recommending PHPs and 84.8% maintaining 12-step program involvement post-completion.<br /><br />Participants commonly valued elements such as 12-step meetings, formal SUD treatment, and drug/alcohol testing, whereas worksite monitoring was often rated least valuable. Costs for participation varied widely—from $250 to $321,000—with most respondents (85%) feeling the financial burden was justified. However, challenges persist, including the financial strain especially for early-career physicians, such as medical students and residents. Stigma related to workplace monitoring may discourage physician-patients from seeking help. Moreover, the absence of federal standardization leads to variability in PHP practices across states.<br /><br />Overall, PHPs represent a successful model for physician recovery, emphasizing structured treatment and ongoing support. Yet, they face obstacles like cost, stigma, and inconsistent regulations that need addressing to improve accessibility and efficacy for all physicians affected by SUDs.
