Development of a Conceptual Model of the Patient Experience of AUD

This targeted qualitative literature review (TLR), sponsored by Eli Lilly and Company, analyzed studies published from 2019 to 2024 to develop a conceptual model capturing the patient experience of Alcohol Use Disorder (AUD). AUD is a prevalent, chronic, relapsing condition marked by impaired alcohol control despite harmful consequences, affecting 27.9 million US individuals aged 12 and older. It imposes significant medical, social, psychological, and economic burdens, severely diminishing quality of life (QoL). Despite its prevalence, fewer than 10% receive treatment, and up to 75% relapse within a year.<br /><br />The TLR reviewed ten qualitative studies involving thematic coding of patient interviews and author interpretations, focusing on AUD symptoms and QoL impacts. The analysis revealed key symptom domains: craving (including compulsion, psychological struggle, emotional and physical sensations), withdrawal (nausea, shivering, cardiac symptoms, seizures, sleep disorder), and alcohol dependency behaviors (inability to stop, denial, drinking incorporation into daily life). Acute intoxication effects included both positive aspects such as relaxation and emotional regulation, and negative effects like fatigue, memory loss, and confrontations.<br /><br />QoL impacts spanned multiple domains: psychological/emotional (depression, guilt, anxiety, stigma), social and relationships (family and partner strain, social isolation), lifestyle problems (criminal/risky behaviors), work/financial (job loss, financial hardship), cognitive impairment, and long-term health deterioration.<br /><br />The conceptual model proposed provides a structured framework to identify and evaluate patient-reported outcome (PRO) measures relevant for clinical trials and treatment development, emphasizing the importance of assessing craving, withdrawal, dependency, and QoL impacts. The authors recommend further qualitative research to validate and expand understanding of these patient-centered concepts across AUD severity levels, aiming to enhance treatment relevance and patient care.<br /><br />In summary, this review underscores the profound burden of AUD on patients and the critical need for meaningful outcome measurement in new therapies to better address symptoms and improve quality of life for those affected.
