This case report discusses the management of withdrawal from "Catnip Cocktail," an emerging illicit recreational drug containing 1,4-butanediol (1,4-BDO), a chemical analogue of gamma-hydroxybutyrate (GHB). Marketed misleadingly as a benign product and sold in gyms and smoke shops since 2015, Catnip Cocktail metabolizes into GHB in the body, producing sedative and euphoric effects due to its CNS depressant action.<br /><br />The patient is a 36-year-old single male with a history of generalized anxiety disorder, social anxiety, opioid and benzodiazepine use disorders, head trauma, and hyperlipidemia. His substance use history includes Kratom (in remission on Suboxone), cannabis, cocaine, MDMA, psychedelic mushrooms, and nicotine vaping. Since 2015, he used Catnip Cocktail recreationally, at times consuming up to four bottles daily.<br /><br />Upon discovery of his Catnip Cocktail use, he was treated for withdrawal symptoms with Baclofen (10 mg daily) and Clonidine (0.1 mg three times daily) for one week, alongside Propranolol (10 mg twice daily) for anxiety. Behavioral modifications, such as avoiding driving past the smoke shop selling the drug and relocating his residence, helped manage persistent cravings. This combined approach led to successful abstinence.<br /><br />Baclofen, a GABA-B receptor agonist, has shown effectiveness in treating GHB withdrawal and use disorder due to similar receptor activity. The case highlights Baclofen’s potential as a practical pharmacological option for treating acute withdrawal from Catnip Cocktail and similar GHB analogues.<br /><br />In conclusion, increased clinician awareness and education about Catnip Cocktail and its treatment options are crucial as its recreational use grows. This report underscores the importance of combining pharmacological and psychosocial strategies for effective withdrawal management.
