Buprenorphine Dosing in Patients with Kratom Use Disorder

This systematic review analyzed data from 18 small studies (2008–2024), encompassing 52 patients with Kratom Use Disorder (KUD) treated with buprenorphine. The studies, mostly case reports and series, examined relationships between buprenorphine dosing and patient demographics including age, sex, kratom dose, and duration of use.<br /><br />Patients ranged from 20 to 65 years old (mean age 38.4), with 65.3% male. Statistical analysis revealed a moderate positive correlation between age and buprenorphine dose (r = 0.434, p = 0.003), indicating that older patients tended to be prescribed higher doses. In contrast, younger patients received a wider range of doses. Age was inversely correlated with duration of buprenorphine treatment (r = –0.327, p = 0.03), meaning older adults used buprenorphine for shorter periods than younger individuals. No significant links were found between buprenorphine dose and sex, kratom dose, duration of kratom use, or treatment initiation timing.<br /><br />The review highlights a lack of standardized dosing regimens for KUD, noting that most patients stabilize on 8–16 mg of buprenorphine, with treatment plans individualized based on clinical response. The timing of initiation is generally recommended 12–24 hours after last kratom use to manage withdrawal effectively. The inconsistencies in kratom product composition and variability limit the ability to establish dose-response patterns and assess treatment efficacy fully.<br /><br />Limitations include small sample sizes, incomplete and inconsistent data reporting, lack of standardized measures for kratom dependence severity, and retrospective study designs without controls, restricting causal conclusions.<br /><br />In conclusion, while buprenorphine shows promise for treating KUD, further research is necessary to evaluate evidence-based dosing strategies, safety, and effectiveness. The study calls for additional prospective research addressing pharmacological approaches to kratom withdrawal to improve clinical guidelines.
