The Youth Opioid Recovery Support (YORS) intervention is a behavioral, multi-component program designed to increase adherence to medication for opioid use disorder (MOUD) and prevent relapse among youth. Initially proven effective in young adults using extended-release MOUD (XR-MOUD), this pilot study expanded YORS to adolescents aged 15-17, including those using sublingual buprenorphine (SL-Bup), the most common MOUD choice in youth despite benefits of XR formulations.<br /><br />The study enrolled 11 adolescents from Maryland Treatment Centers in a 16-week feasibility pilot trial where YORS was adapted by tailoring therapy and family involvement to adolescent needs and incorporating contingency management and monitoring for SL-Bup. Participants, predominantly Black and/or Hispanic and mostly male with a mean age of 15.8 years, all involved a parent in the intervention.<br /><br />Results showed promising medication adherence, with an average of 70% (approximately 79 days) of the 16-week period covered by MOUD. Notably, adherence exhibited a bimodal pattern: six participants had over 90% adherence, while the others were around 50% or less. Four participants switched from SL-Bup to XR-MOUD during the trial. The modified YORS intervention was feasible and yielded higher adherence rates than typically seen in adolescent treatment retention.<br /><br />Limitations include the small sample size, lack of a control group, recruitment of only English-speaking families, and unknown scalability of the intervention. These preliminary findings support further research, leading to an ongoing larger R34 trial with a more diverse sample, a longer duration (26 weeks), and plans for randomized controlled trials to formally assess efficacy and implementation.<br /><br />Overall, YORS shows promise as an adolescent-focused strategy to improve MOUD adherence and treatment outcomes in youth with opioid use disorder, addressing a critical need for evidence-based adolescent OUD treatments.
