A Theoretical Framework for Promotores-Led Interventions in Latinx Problem Gambling

This study examines problem gambling within Latinx communities in California, focusing on the role of promotores de salud—community health workers offering culturally grounded support. Problem gambling affects 3.4% of Californians and is linked to adverse health and family outcomes. Latinx individuals may experience equal or higher rates but underutilize free treatment programs like California's CalGETS due to stigma, limited awareness, language barriers, and a lack of culturally appropriate care.<br /><br />The research involved focus groups with 43 promotores conducted in Spanish, alongside a narrative review of literature from 2000 to 2024. Using community health and structural vulnerability frameworks, the study explored promotores' potential to reduce disparities in problem gambling treatment access and engagement.<br /><br />Key findings highlight that gambling in Latinx communities functions as a socially embedded routine, often involving multiple generations and normalized in various settings. Traditional gender norms, especially machismo, contribute to men hiding gambling problems due to stigma, creating barriers to seeking help. The culture of silence and shame surrounding problem gambling fosters isolation and underreporting.<br /><br />Promotores build trust over time employing culturally-informed methods akin to Motivational Interviewing, acting as trusted community facilitators who reduce stigma, increase awareness, and connect individuals to care. Their embeddedness in the community positions them uniquely to address structural and psychosocial risks linked to gambling.<br /><br />Evidence from related health fields suggests strong potential for promotores-led, gambling-specific interventions. Integrating promotores into gambling prevention and treatment programs may enhance access, engagement, and culturally sensitive support for Latinx populations, ultimately helping to address disparities in problem gambling treatment utilization.
