Design and Implementation of a Clinical Rotation on Harm Reduction

The Yale School of Medicine implemented a clinical rotation focused on harm reduction (HR) to address a gap in addiction education. This rotation is based on the need for practical HR education despite its importance in addiction psychiatry and medicine. The rotation's curriculum development followed Kern et al.’s model, incorporating foundational principles alongside hands-on learning to enhance comprehension and application of HR strategies in clinical settings.<br /><br />Seven addiction psychiatry fellows participated in this program, with pre- and post-surveys assessing changes in their understanding, confidence, and attitudes towards HR. Outcomes showed significant improvements in these areas, particularly in confidence and understanding of HR principles, as well as attitudes towards applying HR in practice. Informal debriefs and qualitative feedback highlighted positive shifts in team-based approaches and collaborative care as part of the rotation experience.<br /><br />The educational goals of the rotation included a deeper understanding of HR principles, increased confidence and practical skills in HR practices, and professional identity formation with HR values. The curriculum emphasized interdisciplinary collaboration, aligning with a whole health model in addiction care, which was reflected in the qualitative feedback where participants reported enhanced empathy and the importance of collaboration with community partners.<br /><br />In conclusion, the clinical rotation on HR demonstrates a practical and effective method to bridge the gap in addiction education by increasing confidence, understanding, and favorable attitudes towards HR implementation. The program's ongoing recruitment aims to include non-physician trainees like social workers and nurse practitioners, aiming to broadly influence addiction care practices.
