Collaborative Care for Co-Occurring Opioid Use and Mental Health Disorders

This study examines the experiences of healthcare providers delivering Collaborative Care in the CLARO trial, which focuses on treating patients with Opioid Use Disorder and co-occurring Depression and/or PTSD. The goal of this research is to identify themes relevant to future dissemination efforts of the Collaborative Care Model in primary care clinics in New Mexico and California.<br /><br />Interviews were conducted with team members involved in the CLARO intervention, including physicians, care coordinators, behavioral health providers, and administrators. The interviews were recorded, transcribed, and analyzed using qualitative software. Themes and sub-themes were identified based on the analysis of the transcripts.<br /><br />The results show that collaborative care has a positive impact on patients with co-occurring disorders, such as providing reassurance, maintaining engagement, reducing risk, facilitating rapid intervention, and managing complex treatment plans. Providers also find that collaborative care improves care team communication and service delivery for these complex patients. However, clinic-level and state policy-level changes are needed to support the adoption and sustainment of this model.<br /><br />The study also highlights the differences between patients with co-occurring disorders and those without, such as more complex and severe illness, difficulty in engaging and retaining them in care, and the need for a more complex treatment course.<br /><br />Based on their findings, the researchers suggest several future directions for collaborative care, including increasing warm handoffs, increasing primary care provider access to psychiatric consultants, ensuring billing reimbursement, and improving the efficiency of the care coordinator role. They also emphasize the importance of demonstrating cost savings and the need for more research on the Collaborative Care Model in the CLARO trial.
