This case report discusses the phenomenon of chemsex, which refers to engaging in sexual activity while under the influence of a cocktail of drugs, specifically GHB (gamma-hydroxybutyrate) and methamphetamine. Chemsex culture is becoming increasingly common in certain cultural settings, particularly among subsets of men who have sex with men. The use of GHB and methamphetamine in combination during sexual activity is meant to intensify and prolong the experience.<br /><br />GHB is a naturally occurring fatty acid that acts as a CNS depressant. It has a high potential for abuse and can lead to dependence with repeated use. Methamphetamine is an indirect agonist at dopamine, noradrenaline, and serotonin receptors. It can cause a range of toxic effects, including tachycardia, agitation, psychosis, and renal failure.<br /><br />The case report describes a 43-year-old male with a history of HIV and multiple other medical and psychiatric conditions. He presented to the emergency department after experiencing a seizure. He had been using GHB and methamphetamine daily for an extended period of time and had run out of GHB prior to the seizure, leading to withdrawal symptoms.<br /><br />The patient was admitted for medical stabilization and underwent withdrawal management using a phenobarbital taper and other medications. After discharge, he relapsed and returned for a second detoxification.<br /><br />The report emphasizes the need for increased understanding of the toxicity, withdrawal, and long-term effects of chemsex on individuals. It also highlights the importance of educating both patients and healthcare providers about harm reduction strategies. Chemsex carries increased risks of HIV and other STDs, as well as mood disorders, substance-induced psychosis, and addiction. It is also important to consider drug interactions between substances of abuse and antiretroviral therapy in HIV-positive individuals.<br /><br />Overall, this case report sheds light on the growing issue of chemsex and the challenges it presents in terms of recognition, management, and prevention.
