This document is a summary of a study that aimed to explore the treatment outcomes of patients with Opiate Use Disorder who primarily use fentanyl compared to those who use other opiates. The study conducted a retrospective chart review of patients at a single VA Medical Center in Philadelphia. The data collected included demographics, urine drug screen results, treatment duration, buprenorphine induction and maintenance dosages, and other substance use information. The study found that fentanyl and fentanyl plus opiate users required increased doses of Suboxone induction and maintenance compared to opiate only users. However, there was no significant difference in the Clinical Opiate Withdrawal Scale (COWS) scores between the cohorts. Treatment duration for the opiate only group was significantly longer than the fentanyl plus opiate group. Some limitations of the study included the inclusion of urine drug screen panels that lacked fentanyl testing and the data being collected from a singular study site in a veteran only population. The study suggests that more research and evaluation of a larger cohort of fentanyl only patients is recommended. The findings of this study imply that a more robust titration of Suboxone induction and maintenance doses in fentanyl users may lead to improved treatment outcomes, including increased retention in treatment and decreased relapse rates.
